We have a fantastic opportunity for a Care Team Leader to join our team based in Bury on a permanent basis, working 37.5 hours per week. This fantastic shift pattern does include some weekend work but with just 3×12.5 hour shifts per week, it allows for an excellent work-life balance!

You will also be included on the on-call rota.

KEY RESPONSIBILITIES

As a Care Team Leader, you will work as part of our team in a service for clients in the achievement of the Trust’s aims and objectives. You will also assist in the management of the Service.

  • Assisting the on-site management team in supervising all care staff and monitoring the care given to our clients. Spend time with clients and monitor service standards.
  • Conduct regular key working sessions with allocated clients and enable them to participate fully in the life of the service, including clients’ meetings. Helping clients achieve the aims of their support plan.
  • To maintain a clear and calm manner whilst managing client risk through assessment, planning, client knowledge and de-escalation as required
  • Helping induct, train, and supervise colleagues and junior members of staff in all aspects of their work in the Home.
  • Writing, updating, and maintaining clients’ Care Plans
  • Completion of documentation, referrals and returns where necessary
  • Liaising with necessary internal / external stakeholders, specialist and agencies keeping them up to date with progress of applications/referrals.
