We are looking for an Adult Support Coordinator to join our Adults Community Team in Huntingdonshire, based in Huntingdon. This is an exciting opportunity to become part of a welcoming and dedicated locality team that is committed to supporting adults to live independently and achieve positive outcomes.

About you

You will:

- Have excellent communication and interpersonal skills, with the ability to build effective relationships with individuals, families, colleagues, and partner agencies.
- Be confident in having honest, respectful, and sometimes difficult conversations to support informed decision-making and manage expectations.
- Produce clear, accurate, and evidence-based assessments, records, and reports.
- Be IT literate and able to use case management systems effectively.
- Have good organisational and time-management skills, with the ability to prioritise competing demands and manage a varied workload.
- Be able to work both independently and collaboratively within a multidisciplinary environment.
- Show resilience, adaptability, and professionalism in a fast-paced social care setting.
- Have an appreciation of working within budget limitations and balancing available resources with individuals’ needs and desired outcomes.
