Band 5 Community Staff Nurse – Gravesham Community Hospital, Gravesend

Job Introduction 

We’re looking for compassionate, motivated Band 5 Community Nurses to join our friendly Gravesend Alliance Team. You’ll be part of a supportive and forward-thinking group of professionals who are passionate about making a real difference in community healthcare. 

Whether you’re newly qualified or already experienced, we’ll help you thrive through our award-winning Preceptorship Scheme – a structured mix of face-to-face sessions, eLearning, and workshops designed to develop your confidence and clinical skills. 

Our base at Gravesham Community Hospital is ideally located just a short walk from St George’s Shopping Centre, surrounded by cafés, restaurants, and great local amenities – making it a convenient and pleasant place to work from. 

This is a full-time role (37.5 hours per week) within a team that provides care 7 days a week between 08:00 – 20:00, ensuring flexibility and balance across shifts. 

A full UK driving licence and access to a vehicle for work purposes is essential. 

Main Responsibilities 

As a valued member of our team, you’ll: 

  • Deliver high-quality nursing care to patients in their homes and community settings.
  • Work both independently and collaboratively as part of a multidisciplinary team.
  • Support patients and families throughout their care journey, promoting dignity, choice, and independence.
  • Use your strong communication and organisational skills to deliver efficient, compassionate care.
  • Contribute ideas to continuously improve our services and ensure every patient experience is a positive one.

The Ideal Candidate 

We’d love to hear from you if you: 

  • Are a Registered Nurse (NMC/1st level registration).
  • Are confident using IT systems and familiar with Microsoft Office (Outlook, Word, Excel).
  • Have a flexible, proactive approach to work and a genuine passion for community nursing.
  • Hold a full UK driving licence and have access to a vehicle for work purposes.
