Education, Health and Care Coordinator – SEND Team

Overview

How you’ll make a difference

Working within a small cluster team, you will be the main point of contact for parents, carers, children and young adults throughout the education, health and care plan (EHCP) or needs assessment process. Using your communication and relationship-building skills, you will ensure families are kept informed of progress and developments, providing advice and support at every stage.

As part of the Education, Learning and Skills service, you will contribute to the delivery of high-quality, coordinated person-centred services that respond to need and promote an early help and preventative approach.

What you will be doing

• You will be responsible for producing high-quality EHCPs following needs assessments or annual reviews. Working closely with children and young people, their families and a range of professionals, you will ensure plans are co-produced, outcome-focused and compliant with relevant legislation.
• Managing a caseload of EHCPs within a geographical area, you will build effective relationships with schools and families to provide advice and guidance on school placements, funding arrangements and available support. 
• When required, you will prepare and present your cases for multi-agency decision-making panels.
• Working collaboratively with colleagues in education settings and partner organisations, you will develop strong professional relationships and act as a key point of contact on matters relating to special education needs and disabilities (SEND) identification, statutory processes, school information and outcomes. 
• You may be asked to chair multi-agency meetings involving senior education leaders, SENCOs, educational psychologists and health professionals.
• Using your organisational skills, you will plan and priorities your workload to meet team targets, ensuring quality assurance processes are consistently followed and high standards of service delivery are maintained. 

What we need from you

• It is essential that you hold a relevant qualification, such as an NVQ Level 3, or have equivalent relevant experience in health or social care.
• You will have previous experience of working with a wide range of people, including families and professionals across education, social care and health services. Experience of working within a SEND or education setting would be advantageous.
• It will be key for you to have an understanding of legislation relating to children and young people with special educational needs and their families.
• Ideally, you will have knowledge of local authority safeguarding policies and procedures, with an understanding of how these are applied in practice to support children, young people and families.

What you need to know

• We have two positions available, 30 hours per week and 25 hours per week.
• Your salary will be pro rated to hours worked.
• Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
• You will be required to travel widely within the authority so you must either have a current valid driving licence and provide a car or have access to an appropriate means of travel.
