Overview
You will make a real difference to vulnerable adults in our community by assisting them in their time of need providing support and promoting their independence.
What will you be doing?
- As a key contact for cases, you will be required to visit service users and their families/carers and undertake an assessment of need.
- You will be assessing for the provision of equipment to support individuals and help prevent, reduce and or delay care and maximise independence under the Care Act 2014
- You will be assessing the need and outlining the specification for adaptations to homes within the eligibility of the Housing Grants, Construction and Regeneration Act 1996.
- In the role, you’ll refer issues if needed in accordance with Safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to profession and Council guidelines.
- You’ll be communicating, providing advice and guidance to service users and providers
- Daily you’ll be liaising and working with Occupational Therapist, Social Workers, Senior practitioners and external contacts.
